Conference on the ‘Social and Health Care Model’

This event is aimed at primary care professionals, geriatricians and professionals from the Aragonese Institute of Social Services, and will present the pharmaceutical and social-health care model proposed following the publication of DECREE 246/2023. The conference will take place on 28 May from 10:30 a.m. to 1:45 p.m. in the ‘Lorente de No’ auditorium of the Aragonese Institute of Health Sciences and is accredited by the CFC.

10:30 a.m.-11:00 a.m. Table 1. Social and healthcare model in our autonomous community.

• Decree on pharmaceutical services, coordination for prescription and supply of medicines in public nursing homes (Ana Cristina Bandrés, URM Unit Coordinator).
• Residential care model and implications for healthcare and support (Mariano Fago, advisor to the IASS Management).

11:00 a.m. - 1:00 p.m. Table 2. What are we doing? Models from a pharmaceutical and social perspective.

• Pharmaceutical care model, validation and pharmaceutical provision from a hospital pharmacy service (Reyes Abad, head of the pharmacy service at Miguel Servet University Hospital).
• Pharmaceutical care model and validation from a Primary Care Pharmacy Service (Arantzazu García, Primary Care Pharmacist, Alcañiz sector).
• Experiences in our community. Work on reducing polypharmacy and withdrawing chemical restraints (Juan Pérez, doctor at the Alagón nursing home).
• Experiences in another autonomous community. Work on monitoring and advising on prescriptions (Idoia Beobide, head of pharmaceutical care at the Matia Foundation).
