Technical Conference “From supporting needs to supporting lives: Case management in long-term care”
On Human Rights Day, we remind ourselves that care is a collective responsibility, and we do so at an event organised by the Official College of Social Work of Asturias, in collaboration with CECOEC through the Oviedo Chamber of Commerce.
Community social work, the promotion of networks and collaboration between services and systems are essential to address challenges and support meaningful lives for people throughout their life course. It is necessary to recover methodologies that promote person-centred work and move towards personalised approaches.
Therefore, based on the Local Care Ecosystems of Guipúzcoa, the working day focuses on case management, which is key to coordinating care for people with complex needs, ensuring that services adapt to people's needs over time.
With the help of our colleagues Maider Azurmendi, head of Community Networks, and Erkuden Aldaz, director of Matia Institute, we have delved into case management as a key methodology for:
- Coordinating support effectively.
- Ensuring comprehensive and personalised care.
- Moving towards a person-centred care model.
