Artikulu honetan Gipuzkoako Foru Aldundiko Gizarte Politikako Departamentuak Matia Institutu Gerontologikoarekin lankidetzan bultzatutako proiektua laburtu da. Proiektuaren helburua hauxe da: familia inguruneko zaintzetarako prestazio ekonomikoa (FIZPE) jasotzen duten pertsonei jarraipena egiteko eta mendeko egoeran dagoen pertsonak jasotzen duen zaintzaren kalitatea eta zaintzailearen bizi kalitatea bermatzeko eredu bat planteatzea.

This article summarises a project promoted by the Department of Social Policy of the Provincial Council of Gipuzkoa, in collaboration with the Matia Gerontological Institute, with the aim of proposing a model for monitoring people receiving financial assistance for care in the family environment (PECEF), and for ensuring both the quality of the care that the dependent person receives and the quality of life of the caregiver in those situations.

Artikulu honetan, pertsonarengan ardaztutako arreta eredua ezartzearen ondorioak ebaluatu dira, Matia Fundazioko 232 profesionalez osatutako lagin baten adierazpenak aintzat hartuta.

Esku hartze horiek Etxean Ondo Egoitzak proiektuaren barruan ezarri ziren eta, horiek ebaluatzeko, Person-centered Care Assessment Tool (P-CAT) eta Staff Assessment Person Directed Care (PDC) galde sortak banatu ziren.

This article presents the results of an evaluation to identify the effects of interventions associated with the People-Centred Care Model in a sable of 232 professionals from Matia Fundazioa.

Interventions were implemented in the Etxean Ondo Nursing Homes and, to evaluate them, the Person-Centred Care Assessment Tool (P-CAT) and Staff Assessment Person Directed Care (PDC) questionnaires were administered.

La Atención Centrada en la Persona (ACP) es considerado un enfoque de gran valor para mejorar la calidad de los cuidados a las personas mayores (Nolan, Davies, Brown, Keady, & Nolan, 2004). Es considerado en la actualidad como: a) un eje fundamental en el diseño de los servicios sanitarios y sociales; b) un eje fundamental en la evaluación de la calidad asistencial; y c) el principal referente para el movimiento por el cambio de modelo en residencias de personas mayores.

Pertsonarengan ardaztutako arreta (PAA) adinekoei zuzendutako zaintzen kalitatea hobetzeko ikuspegi garrantzitsutzat jotzen da (Nolan, Davies, Brown, Keady, 2004). Izan ere, aditu askoren ustez, eredu hori a) osasun eta gizarte zerbitzuak diseinatzeko funtsezko ardatza da; b) laguntzaren kalitatea ebaluatzeko ezinbesteko oinarria da eta c) zahar etxeen eredua aldatzeko mugimenduaren erreferentzia nagusia da.

Person-Centred Care (PCC) is considered a valuable approach for improving the quality of care for the elderly (Nolan, Davies, Brown, Keady, & Nolan, 2004). It is now regarded as: a) a cornerstone in the design of health and social services; b) a cornerstone in evaluating the quality of care; c) the main reference for the movement for change in the nursing home model.

Que las personas mayores con necesidad de apoyos y atención y sus familias cuidadoras los reciban de manera integral, coordinada y continuada y en las mejores condiciones para su bienestar y calidad de vida. Que mediante estas experiencias piloto, que utilizan metodologías como gestión de casos, atención integral y centrada en la persona, se genere conocimiento sobre la eficacia y eficiencia del modelo aplicado.

So that older people in need of support and care and their family caregivers receive it in a comprehensive, coordinated and sustained manner and in the best conditions for their wellbeing and quality of life. So that through these pilot projects, using methodologies such as case management, comprehensive and person-centred care, knowledge about the effectiveness and efficiency of the applied model is generated.
