Azpeitia care ecosystem

In Azpeitia, a process was launched in 2024 to move towards a local care ecosystem, with the aim of ensuring that everyone living in the municipality has access to continuous and consistent care pathways, making it easier for everyone to carry out their life plans.

So far, an assessment of the care system in Azpeitia has been carried out. This has identified needs and opportunities from the different parts that make up the care ecosystem: the community, services, institutions, people who need support and carers.

At the same time, the municipality's Social Services have begun to incorporate case management methodology into their daily practice. During 2026, the aim is to accompany some people by applying this methodology, which involves continuous monitoring and designing a personalised care plan. This approach will provide a better understanding of each person's situation and enable more effective coordination of the response between the different resources available.

In addition, throughout 2026, an action plan -ZAINTZA PLANA- will be co-created to consolidate this working model and define the steps to be taken in the coming years.
