SUSTRAIAK – Advancing in Person-Centred Practices

Sustraiak is a project by Matia aimed at transforming the care model towards Person-Centred Care (PCC), ensuring that each individual can make decisions about their life and support based on their values, wishes, and needs. The project combines changes in the organizational structure, the development of new Person-Centred Planning (PCP) tools, and the creation of shared learning spaces among residents, families, professionals, and the community.
Objectives

Move towards a more humane, participatory, and personalized care model, placing the person at the centre of all decisions.

Implement new organizational roles that strengthen closeness, participation, and continuity in support.

Develop and implement practical PCP tools that enable consistent application of the model in everyday life.

Methodology

Pilots in real centres: progressive implementation in Lamourous and Bermingham, adapted to the context and pace of each centre.

Creation of new organizational roles: Housing Manager and Housing Representative.

Design and testing of PCP tools: personal profile, personal plan, support circle, contribution sheet, learning log.

Training pathways: targeted at professionals, residents, and families, with both theoretical and experiential sessions (human rights, ethics, person-centred relocation, conflict management, community).

Co-creation spaces: multidisciplinary working teams and participation of external stakeholders (e.g., collaboration with Uliazpi).

Evaluation and adjustment: recording learnings and adapting methodologies based on practical experience.
Results
Results so far

Pilot implementation in Lamourous (9 housing units and Day Centre) and Bermingham (2 housing units), at different stages of development.

Creation and rollout of the roles of Housing Manager and Housing Representative.

Design of training pathways for professionals, residents, and families.
