Towards a standards-based model of care and support in Gipuzkoa

The reference framework for the standards-based care and support model presented here enables a comprehensive assessment of the outcomes of the practices offered by long-term care services, and an evaluation of the extent to which they are progressing towards greater personalisation of care.
Its application should enable us to identify lessons learnt across the network as a whole, allowing us to continue optimising the quality of care by objectively measuring outcomes for individuals.
This framework is the result of a process of analysis and consensus-building led by the Comprehensive Centre Management Service of the Department of Care and Social Policies of the Provincial Council of Gipuzkoa, in collaboration with Matia Fundazioa, and has involved experts by experience, frontline care professionals, service managers and technical staff from the Department.
