Reference framework for the care and support model in Gipuzkoa based on standards

In recent years, various institutions have been developing strategies, plans and tools aimed at transforming care models, with a particular emphasis on long-term support and care. There is a consensus, both at European and national level, supported by evidence and results obtained in different countries, on the need to promote and develop practices based on a rights-based approach and respect for human dignity, which take into account the uniqueness of individuals, respect their self-determination in terms of how they want to live their lives and receive care, and aimed at achieving results in terms of improving well-being and quality of life.
This document proposes a reference framework for the care and support model in Gipuzkoa, focusing on the evaluation of person-centred outcomes. The aim is to transform the care paradigm, moving from an institutional approach to one that prioritises the individuality, self-determination and well-being of users. To achieve this, the report establishes five guiding principles and twenty quality standards that will guide the design, management and evaluation of services.
In addition, it identifies key elements of the system that need to be adjusted, such as governance, legislation, professionals and policies, to ensure inclusive and community-centred care, promoting autonomy and active participation of individuals.
